  • Enclosed cargo area, impervious to leaks/spills
  • Manifest must be kept on truck
  • Scale must be on vehicle
  • Permit No Decal. On left door, Address, Phone no., Permitted Medical
  • Waste Hauler 2inch or taller, Biohazard symbol signage
  • Vehicle secure when unattended
Manifest Requirements
  • Generator name, address, phone, and permit no.
  • Transporter name, address, phone, and permit no.
  • Tx facility name, address, phone and permit no.
  • Type and quantity of waste
  • Date collected/date picked-up or delivered
  • Kept for 3 years
Storage Requirements
  • 45 degrees F if stored more than 72hrs
  • Cannot be stored more than 30 days regardless of temp
